ALL VISITORS:

1. Read the CT Horse Business Statutes.

2. Print the Liability Waiver. Complete the form, sign and date. Scan and email the form to Psi Equine at cory.b.sells@gmail.com. Keep the hard copy for your records.

Each Adult must complete this form. Multiple children can be listed on the adult's form (adult must be the parent or guardian of each child listed).

PSYCHOTHERAPY CLIENTS:

Do not send any forms through email as it is not confidential or secure. Please print and complete forms to bring with you to your first session. You will need the Liability Waiver and The Informed Consent. The Release of Information Form is coming soon, or you may use request one from your psychothterapist.
